Job Responsibilities
- Promotes optimal clinical and resource utilization and facilitates appropriate lengths of stay and reimbursement for hospital admissions.
- Acts as a key informational and educational resource liaison for the team.
- Develops organizational approaches to problem-solving.
- Analyzes current systems and variances to identify improvement opportunities.
- Promotes quality of care through collaboration with service team members, patients, and families.
- Oversees discharge/transition planning executed by Care Management staff and audits activities for accuracy.
- Conducts weekly Clinical-Financial meetings to monitor patients with high dollar, high length of stay, and/or social barriers.
- Collaborates with multidisciplinary teams to problem-solve complex cases.
- Demonstrates clinical expertise and serves as a resource and role model for staff.
- Provides guidance on complex patient assessments and interventions.
- Documents interactions with patients and families in a timely and comprehensive manner.
- Assists executive leadership in developing annual competency analysis system requirements.
- Provides feedback to the Manager on employee performance.
- Supports an innovative and empowering care delivery environment.
- Promotes effective communication and conflict resolution skills within the staff.
- Performs monthly compliance audits as directed.
- Educates hospital staff on payer regulations and managed care principles.
- Fosters the integration of staff and/or students into the healthcare team.
- Participates in self-development activities to improve management skills.

Requirements
Experience: Five (5) years of Medical Social Work or Nursing experience, with 2 years in an inpatient hospital setting and 2 years of Care Management or supervisory experience
Education: Master’s Degree in Social Work or Bachelor of Science Degree in Nursing within six months of hire
License & Certification:
- LGSW/LCSW/LICSW certification in West Virginia
- Current unencumbered licensure as a registered nurse in West Virginia or through the enhanced Nurse Licensure Compact (eNLC)
