Implements and monitors patient care plans
Monitors, records, and communicates patient condition as appropriate
Serves as a primary coordinator of all disciplines for well-coordinated patient care
Notes and carries out physician and nursing orders
Assesses and coordinates patient's discharge planning needs with members of the healthcare team
Conducts initial assessment and re-assessment of patients
Identifies problems based on patient responses to health conditions or needs
Sets measurable and achievable goals in collaboration with the patient and family
Establishes daily goals with the patient and family
Implements nursing care according to the care plan and documents in healthcare record
Ensures patient safety and medication administration
Provides ordered treatments and education to patients and families
Evaluates the patient's status and modifies care plan as needed
Coordinates care and manages the effectiveness of patient care from admission to discharge
Delegates and supervises nursing tasks
