Job Responsibilities

Coordinate and oversee all direct care patient services provided by clinical personnel.
Develop, plan, implement, analyze, and organize clinical operations for the managed location.
Conduct and delegate assessment and reassessment of patients and update care plans.
Manage assignment of caregivers and oversee delivery of care to all patients served.
Review available patient information related to cases and determine home health or hospice needs.
Ensure patients meet admission criteria and make admission decisions.
Assign appropriate clinicians to cases as needed.
Instruct and guide clinicians to promote effective performance and quality home care services.
Monitor cases to ensure documentation compliance with regulatory agencies and third-party payers.
Coordinate communication between team members, attending physicians, and caregivers.
Work with the Branch Director and Company Finance Department on revenue and budget goals.
Participate in sales and marketing initiatives.
Supervise clinical employees at the specific location.
Handle employee corrective action and discipline issues fairly and objectively.
Participate in interviewing, hiring, training, and development of direct care clinicians.
Evaluate clinician performance relative to job goals.
Assess staff education needs and conduct regular staff education as needed.
Analyze situations and identify problems, evaluating alternative courses of action.
Review clinical staff documentation for appropriateness of care, delivery, and documentation requirements.
Oversee QA/PI activities and work with Utilization Review staff on performance review.
Provide direct patient care in emergencies.
Act as Branch Director in his/her absence.
Ensure compliance with Company standards and external regulatory authorities.
Maintain relationships with referral/community sources and participate in professional organizations.

Required Skills

Core Skills

Registered Nurse license
Management experience in home care or hospice
Strong communication and leadership skills
Ability to analyze clinical situations

Preferred Skills

Experience with regulatory compliance
Knowledge of Performance Improvement principles
Experience in training and development

Requirements

Experience: Two years as a Registered Nurse with at least one year of management experience.

Education: Graduate of an accredited School of Nursing.

License & Certification:

Registered Nurse license
CPR certification
Valid driver’s license
