Job Responsibilities

Utilizes the nursing process to address physical, psychological, emotional, and social needs of patients.
Collaborates and communicates with primary care providers regarding patient care and management.
Supports implementation of clinical care pathways for common primary care conditions.
Participates actively in quality improvement opportunities.
Provides patient education for preventive care, chronic disease management, and medication management.
Documents all patient interactions and clinical care in electronic medical records (EMR).
Performs care coordination for patients, including abnormal results and triage.
Minimizes risk of harm to patients by following established nursing practices and protocols.
Uses information technology to communicate and manage knowledge.
Monitors outcomes of care processes and supports ethical decision making.
Functions effectively within inter-professional teams for quality patient care.
Creates an environment of belonging and inclusion.
Actively participates in team huddles and provides updates.

Required Skills

Core Skills

Current RN licensure in state of practice
BLS certification
Evidence-Based Practice
Clinical expertise in ambulatory care

Preferred Skills

1+ years' experience in an ambulatory setting
Encouraged to obtain ambulatory care nursing certification

Requirements

Experience: 1+ years’ experience as a Nurse, in an ambulatory setting preferred.

Education: Graduate of an accredited school of nursing required; BSN or completion of BSN or appropriate advanced degree within 3 years of hire preferred.
