Job Responsibilities
- Utilizes the nursing process to address the physical, psychological, emotional, and social needs of patients.
- Collaborates with providers and support staff to provide clinical nursing interventions.
- Communicates effectively and respectfully with patients to address individualized needs and priorities.
- Integrates Evidence-Based Practice and patient preferences for optimal healthcare delivery.
- Collaborates and communicates with primary care providers regarding patient care and management.
- Supports implementation of clinical care pathways for common primary care conditions.
- Actively participates in quality improvement opportunities.
- Utilizes evidence-based ambulatory care nursing interventions during clinical visits and electronic communications.
- Educates patients and their families on preventive care and chronic disease management.
- Documents all patient and family interactions in the electronic medical record (EMR).
- Performs care coordination for patients, including triage and management of abnormal results.
- Oversees clinical care essential to the promotion and restoration of patient health.
- Minimizes risk of harm to patients by following established nursing practices.
- Uses information technology to manage knowledge and mitigate errors.
- Monitors outcomes of care processes to support ethical decision-making.
- Functions effectively within primary care inter-professional teams.
- Creates an environment of belonging and inclusion.
- Participates in team huddles/assemblies for clinical operations updates.
- Demonstrates willingness to give and receive feedback.
Required Skills
Core Skills
- Registered Nurse licensed in the state of practice
- Ability to perform ambulatory care nursing interventions
- Knowledge of clinical care pathways
Preferred Skills
- Bachelor of Science in Nursing
- Experience in an ambulatory setting
Requirements
Experience: 1+ years’ experience as a Nurse in an ambulatory setting preferred
Education: Graduate of an accredited school of nursing required; Bachelor of Science in Nursing strongly preferred
License & Certification:
- Current RN licensure in state of practice
- BLS required based on organizational guidelines/policies
