Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial assessments, re-assessments, and focused assessments based on patient condition, setting, and population.
Identifies problems based on clinical judgment about the patient’s response to health conditions or needs.
Plans and sets measurable and achievable goals in collaboration with the patient and family.
Documents assessment data, problem identification, and goals in the patient’s care plan.
Establishes daily goals with the patient and family.
Implements nursing care according to the care plan, ensuring continuity during hospitalization and discharge.
Ensures patient safety through medication administration and provision of ordered treatments.
Educates patients and families about care and documentation.
Continuously evaluates patient status and effectiveness of nursing care, modifying the care plan as needed.
Coordinates care from admission to discharge.
Delegates and supervises tasks as needed.
Prioritizes nursing care activities.
