Job Responsibilities
- Facilitate advanced illness management and palliative care for patients.
- Ensure a comprehensive, individualized treatment plan.
- Provide diagnosis, treatment, consultation, and follow-up under the direction of the Palliative Medical Director or Primary Care physician.
- Conduct initial assessments, including disease-specific changes, patient function, nutrition and cognition.
- Establish the individualized plan of care in consultation with the patient, family, and Advanced Illness team.
- Facilitate obtaining care for patients in accordance with the Plan of Care.
- Participate in evening/weekend call, providing clinically competent and responsive on-call services.
- Prescribe medications, including controlled substances, as delegated and licensed.
- Order and interpret laboratory, imaging, and other diagnostic tests.
- Communicate clinical findings to the PSC Physician as needed, at least bi-weekly.
- Re-assess effectiveness of care plan regularly and modify as needed.
- Maintain effective working relationships with the supportive care team.
- Communicate and document clinical findings, treatment plans, and care provided.

Requirements
Experience: Minimum of two years Nurse Practitioner experience
Education: Minimum of Masters of Science in Nursing from an accredited school of nursing or equivalent as allowed under state law
License & Certification:
- Current license as a registered nurse or equivalent as required by state law
- Current Nurse Practitioner certification/license
- Collaborative agreement with a supervising physician
- Part B Provider certification
- Federal DEA license
- State DEA license (if required)
- ACHPN – Certification in Hospice and Palliative Care (Preferred)
Schedule:
- Evening/Weekend Call as required
