Job Responsibilities
- Collaborate with the Interdisciplinary Team to assess the needs of the resident.
- Provide interdisciplinary schedule for MDS assessments and care plan reviews as required by governing agencies.
- Ensure decisions to complete or not complete additional MDS assessments are based on clinical criteria as identified in the most recent version of the RAI User’s Manual.
- Assist with coordination and management of the daily stand-up meeting, including review of resident care and setting of assessment reference dates.
- Comply with federal and state regulations regarding the completion and coordination of the RAI process.
- Monitor MDS and care plan documentation for all residents; ensure documentation supports MDS coding.
- Maintain current MDS status of assigned residents according to state and federal guidelines.
- Perform accurate data entry of resident information into appropriate computerized MDS programs.
- Complete accurate coding of the MDS using information obtained from medical record review, observation, and interviews with facility staff, residents, and family members.
- Attend interdisciplinary team meetings, quality assurance meetings, and other meetings to gather information and maintain and update records.
- Assist DON or designee with identification of significant changes, physician orders, and verbal reports to ensure MDS and care plans reflect those changes.
- Prepare scheduling and notices of resident care planning conferences, and assist DON in communicating outcomes/problems to responsible staff, residents, and/or responsible parties.
- Attend weekly Medicare, Medicaid, and Managed Care meetings as required.
- Correct and ensure the completion of final MDS and submit resident assessment data to the appropriate State and Federal government agencies.
- Assign, assist, and instruct staff in the RAI Process, PPS Medicare, Medicaid (Case Mix as required), and the clinical computer system in relation to these processes.
- Maintain confidentiality of necessary information.

Required Skills
Core Skills
- Registered Nurse or Licensed Vocational Nurse license in the state of practice
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility
- Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement
- Thorough understanding of the Quality Indicator process
- Knowledge of OBRA regulations and Minimum Data Set
- Knowledge of the care planning process
- Experience with MDS 3.0

Requirements
Experience: Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
Education: Graduate of an approved RN/LVN program and licensed in the state of practice required.
