Job Responsibilities
- Accurate and timely completion of all Medicare/Medicaid case-mix documents.
- Conduct continual Minimum Data Set (MDS) reviews.
- Oversee the overall process and tracking of MDS/Prospective Payment System (PPS) documentation and submission.
- Integrate nursing, dietary, social recreation, restorative, rehabilitation, and physician services for appropriate assessment and reimbursement.
- Assess and determine the health status and level of care of all new admissions.
- Ensure accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, and significant changes.
- Communicate level of care for new residents to all disciplines.
- Coordinate interdisciplinary participation in completing the MDS for each new admission.
- Maintain an accurate schedule of all MDS assessments.
- Responsible for accurate data entry and electronic submission of MDS assessments.
- Verify electronic submissions of MDS and perform corrections when necessary.
- Schedule and conduct resident care conferences in compliance with regulations.
- Assist disciplines in formulating and revising care plans.
- Evaluate resident care plans for comprehensiveness and individuality.
- Assess the achievement of desired outcomes and revise care plans as needed.
- Responsible for all level of care changes within the facility.
- Notify all departments of any level of care changes.
- Generate appropriate forms for level of acuity and changes.
- Transmit forms to the appropriate agency for processing.
Required Skills
Core Skills
- Registered Nurse with current unencumbered state licensure
- Long Term Care Experience (preferred)
- Ability to read, write, speak and understand the English language
