Job Responsibilities
- Provide and facilitate comprehensive care management and quality outcomes for patients across the healthcare continuum.
- Focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and length of stay.
- Directly support hospital patients identified in need of comprehensive case management.
- Promote effective utilization and monitoring of health services.
- Collaborate and communicate with the healthcare team and patient/caregiver to manage care and transitions.
- Develop and/or implement a comprehensive care plan based on assessment and evaluation, addressing physical, mental, and psychosocial needs of the inpatient/caregiver.
Required Skills

- Experience in acute care case management
Requirements
Experience: 12 months nursing experience; 1 year acute care case management preferred
Education: BSN preferred
License & Certification:
- Registered Nurse
- Basic Life Support (within 90 days)
- Case Management certification (ACM, CCM, CCCTM, CMAC or CMGT-BC) required within one year of eligibility
