Job Responsibilities

  • Screen/assess appropriate referrals for admission and discuss potential for admission with the staff.
  • Direct and process admissions and serve as the liaison between the Directors of Admissions, hospitals, admitting/attending physicians, and other referral sources.
  • Develop a system for coordinating and monitoring resident assessments and referrals.
  • Conduct and document nursing assessments as necessary.
  • Complete and submit admission assessment reports as requested.
  • Coordinate admissions with necessary staff to ensure a smooth flow of information and activity.
  • Serve as a point-of-contact for referrals/inquiries and conduct initial clinical review as indicated.
  • Process referrals and inquiries.
  • Work with other facility departments to ensure an accurate and timely admission process.
  • Maintain an accurate record of available beds/units.
  • Communicate the care needs and medical equipment needs of residents upon admission with the Director of Admissions and assist as indicated.
  • Conduct rounds and provide information to appropriate staff to ensure readiness for admissions.
  • Obtain necessary medical records for admission, i.e., diagnosis, physician orders, OBRA screening, etc.
  • Assume additional responsibilities and special projects as assigned or needed.
  • Ensure admission assessments and orders are obtained, physician to physician communication occurs where necessary, and that the resident’s admission assessment process is managed efficiently and effectively.
  • Ensure that the resident/family understands level of care and expectations.
  • Ensure that information about resident progress is communicated as needed with staff, physicians, and referral sources.
  • Ensure good public relations with the general public, residents, families, and guests, to promote a positive image.
  • Ensure that residents and their representatives/family members are oriented to payment source/rates, billing procedures, visitor/guest privileges, and care procedures as appropriate.
  • Treat all residents fairly, with kindness, dignity, and respect.
  • Work with residents, physicians, nursing staff in a polite, courteous, and professional manner at all times and provide support to residents and their families.
  • Maintain a clean and safe work environment.
  • Maintain confidentiality of resident and organization information and respect residents’ privacy and dignity.
  • Cooperate with other staff members to achieve departmental and organizational goals and promote good relations.
  • Act as a role model for good facility customer service.
  • Adhere to State and Federal regulations governing Long Term Care.
  • Participate in staff development programs and staff meetings.
  • Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment.
  • Comply with organizational policies and procedures.
  • Report unsafe/hazardous conditions and/or defective equipment immediately.
  • Maintain a neat, professional appearance and adhere to dress code.
  • Comply with Corporate Compliance Program.
  • Participate in facility and divisional meetings as requested.
  • Report all allegations of resident abuse, neglect and/or misappropriation promptly to Executive Director.
