Pay: $39.21 - $72.83 Hourly

Shift: 9:00 AM - 5:30 PM

Job Responsibilities

  • Coordinates and directs level of care appropriateness from admission throughout the entire stay.
  • Uses discretion and independent judgment in applying screening criteria standards and maintaining compliance.
  • Prioritizes and delegates daily functions to assure completion of required tasks.
  • Completes planning using high risk admission data and discharge criteria.
  • Anticipates medical and psychosocial needs in high risk patients.
  • Screens new patients daily and collaborates with Social Worker on cases.
  • Collaborates with Care Coordination assistants for administrative needs.
  • Manages level of care status through continued stay reviews, removing care barriers, and revising transition plans.
  • Ensures all appropriate regulatory documents are provided to patients.
  • Completes discharge needs assessments and formulates plans on all admissions.
  • Acts as a clinical role model to the healthcare team.
  • Leads and facilitates Multidisciplinary rounds for patient care progression.
  • Addresses readmission prevention strategies and LOS concerns.
  • Queries healthcare team members for accurate clinical documentation.
  • Coordinates to ensure implementation of discharge plans.
  • Refers cases not meeting admission criteria to physician advisors.
  • Proactively interacts with third party payers to prevent denials.
  • Acts as a liaison between patients, physicians, families, and healthcare teams.
  • Provides education to healthcare team members.
  • Provides a concise hand-off to colleagues daily.
  • Tracks and trends variance data and elevates concerns to management.
  • Complies with all standards and policies regarding patient safety initiatives.
  • Fulfills all compliance responsibilities related to the position.
  • Performs other duties as assigned.

Required Skills

Core Skills

  • Registered Nurse license in the State of CT
  • Minimum of three years job-related experience
  • Knowledge of electronic medical records
  • Expertise with Interqual and Milliman screening criteria
  • Excellent collaboration and communication skills

Preferred Skills

  • Certification in Case Management preferred or willingness to obtain
  • Bachelor's Degree in Nursing preferred

Requirements

Experience: Minimum of three years job-related experience

Education: Associate Degree in Nursing

License & Certification:

  • Registered Nurse license in State of CT
