Position: Nurse for Homecare Services

Job Type: Full Time

Pay: $30-$37 per hour

Shift: Saturdays 9A - 5P, Sundays 10P - 7:15A

Provide care that is based on licensure/certification within the scope of practice.
Maintain an updated clinical record on each patient at all times, meeting required deadlines for documentation.
Evaluate the effectiveness of nursing service to the patient and family on an ongoing basis.
Consult with physicians concerning alterations of patient care plans and report all changes in care to the nurse supervisor.
Mentor and/or coach new employees during patient-specific orientations.
Educate patients and their caregivers on topics related to patients' care needs.
Participate in FFHC Quality Assurance, Performance Improvement program (QAPI) as requested.
Strive to improve nursing care skills continuously.
Attend and/or complete all mandatory training/in-services as assigned.
Submit required personnel file documentation in a timely manner.
Adhere to FFHC compliance and ethics expectations.
Maintain patient and family confidentiality.
Maintain professional, supportive, and responsive interpersonal communication skills.
Perform other duties as assigned.

LPN/LVN Nursing Degree
Valid and unencumbered LPN/LVN License in the state of employment or Multi-State license
1 year of demonstrated experience
Valid CPR card/certificate with hands-on skills check
Ability to read and communicate in English both verbally and in writing
Working knowledge of Nurse Practice Acts

Experience: 1 year of demonstrated experience

Education: LPN/LVN Nursing Degree
