Job Responsibilities
- Assist patients in navigating post-acute care to create a positive impact on patient outcomes and referral source satisfaction.
- Integrate evidence-based clinical guidelines and preventative guidelines into patient-centered transition plans to promote quality and efficiency in post-acute care delivery.
- Represent area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians, and executive level opportunities.
Required Skills
Core Skills
- Basic demonstrated technology skills, including operation of a mobile device.
Preferred Skills
- Registered nurse or physical therapist.
- Three years of field experience.
- Previous experience in home health or healthcare sales.
Requirements
Experience: Two years of demonstrated field experience.
Education: Graduate of an approved school of nursing, therapy, or social work.
License & Certification:
- Valid state driver license.
