Job Responsibilities

Administer medications and monitor vital signs.
Perform wound care and educate patients and families about healthcare needs.
Develop care plans in collaboration with physicians.
Ensure patient safety and comfort.
Document patient care and coordinate with other healthcare providers.
Provide excellent patient care by facilitating collaboration, development, implementation, revision, and reporting of the case management program.
Act as a liaison between the patient, family, nurse, physicians, multidisciplinary team, and patient's healthcare benefactor to optimize outcomes.
Lead the coordination of patient care with other disciplines within the care team.
Monitor the appropriateness and timeliness of care.
Ensure the interdisciplinary care plan is consistent with the patient’s clinical course, continuing care needs, and covered services.
Discuss resource utilization, consultations, treatment plans, estimated length of stay, and discharge plans with physicians.
Focus on complex patients, frequent emergency department utilizers, chronic pain and substance abuse patients, and homeless patients.
Collaborate with acute care case managers to ensure appropriateness of ongoing care.
Coordinate the transfer of patients to tertiary centers, including the transfer of patient information for continuity of treatment and services.
Provide oversight and collect data for regulatory/accreditation compliance.
Manage frequent emergency department visitors by conducting focus studies on previous and current admissions.
