Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial assessment, re-assessment, and focused assessment based on patient condition, setting and population.
Identifies problems based on clinical judgment about the patient’s response to actual or potential health conditions or needs.
Plans and sets measurable and achievable goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Formulates a plan of care.
Implements nursing care according to the care plan to ensure continuity of care.
Ensures patient safety.
Administers medications and provides ordered treatments.
Educates patient/family.
Documents care in healthcare record.
Evaluates the patient’s status and the effectiveness of nursing care, modifying the care plan as needed.
Coordinates care and manages sequences, timing, and effectiveness of patient care from admission to discharge.
Delegates and supervises nursing responsibilities.
Prioritizes patient care tasks.
