Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Collects and analyzes data about a patient, considering physiological, psychological, sociocultural, spiritual, economic, and lifestyle factors.
Conducts initial assessments and reassessments.
Identifies problems and sets measurable short- and long-range goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Formulates a plan of care and ensures continuity of care.
Documents care in the patient’s record and ensures patient safety.
Administers medications and provides ordered treatments.
Educates patients and families.
Evaluates patient's status and the effectiveness of nursing care continuously.
