Job Responsibilities
- Assigned to a population of patients and provides individualized and comprehensive care to reduce barriers and coordinate diagnostic and treatment efforts.
- Ensures prompt access to care.
- Applies the nursing process to perform clinical assessments; education on disease process, plan of care and treatment modalities; and symptom management.
- Collaborates and communicates effectively with the multidisciplinary team to coordinate patient care and ensure continuity of care.
- Creates a comfortable and exceptional patient experience.
- Initiates referrals to appropriate interdisciplinary teams for physical, psycho-social and spiritual support.
- Provides coordination of care and support for patients, families and caregivers throughout the continuum of care.
- Educates patients, caregivers, and families on disease process, plan of care, treatment, symptom management and survivorship.
- Effectively communicates with patients, caregivers, families and multidisciplinary healthcare teams.
- Gathers pertinent patient's medical history and reviews presenting symptoms.
- Demonstrates professionalism in the workplace with patients, families, healthcare team and community.
- Provides excellent customer service resulting in patient satisfaction with care.
- Promotes a patient-and family-centered care environment for ethical decision-making and advocacy for patients.
- Welcomes patients and family members with proper introductions to self and multidisciplinary team members.
- Communicates time for appointments, procedures and potential delays.
- Prioritizes incoming phone messages and triages according to clinical needs in a timely manner.
- Returns calls within the same day of call.
- Uses positive service recovery techniques.
- Returns patient messages and/or requests within 24 hours.
Required Skills
Core Skills
- ADN Completion of accredited RN program
- 3 years of experience as an RN in an inpatient or outpatient setting
- Arizona RN license
- American Heart Association BLS certification
- Computer skills including EMR documentation and Record/Verify system
- Experience in patient education/interventions and symptom management
Preferred Skills
- BSN
Requirements
Experience: 3 years of experience as an RN in an inpatient or outpatient setting
Education: ADN from an accredited RN program
License & Certification:
- Arizona RN license
- American Heart Association BLS certification
