Job Responsibilities
- Plan, organize, and direct hospice care utilizing the nursing process of assessment, planning, interventions, implementation, and evaluation.
- Effectively interact with patients, caregivers, families, and other interdisciplinary team members while maintaining standards of professional nursing and clinical competency.
- Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
- Partner closely with the Clinical Director to assess the eligibility and suitability of clients for specialized hospice services.
- Coordinate care and serve as a source of comfort and support for patients.
- Deliver top-notch nursing care rooted in proven principles and techniques.
- Empower patients and families through education and encourage their active participation in creating personalized care plans.
- Share the responsibility of call duties.
Required Skills
Core Skills
- Strong nursing assessment and intervention skills
- Compassionate patient interaction
- Effective communication with interdisciplinary team
- Computer proficiency, including EMR background
Preferred Skills
- Hospice/home health experience
- Experience as a professional nurse within the last three years
Requirements
Experience: One (1) year experience as a professional nurse within the last three (3) years.
Education: Bachelor's degree in nursing from a program accredited by the National League for Nursing.
License & Certification:
- Current unencumbered license as a Registered Nurse
- Current CPR certification
