Job Responsibilities

  • Promote patient-centered care by coordinating the plan of care for the patient stay.
  • Manage the length of stay and ensure appropriate resource management.
  • Develop a safe and appropriate discharge plan in collaboration with the multidisciplinary team.
  • Facilitate the progression and transition of care using established criteria.
  • Provide case management services for both inpatient and observation patients as assigned.
  • Identify patients who are at risk for adverse outcomes during the transition from one level of care to another.
  • Perform a comprehensive assessment of psychosocial, medical, and discharge needs of patients and families.
  • Reassess the patient’s clinical condition as indicated and develop strategies to mitigate readmission risks.
  • Coordinate the plan of care by collaborating with the multidisciplinary healthcare team.
  • Partner with Social Services to ensure the post-acute medical needs and level of care are appropriate.
  • Involve patients and families in identifying needs and expectations to develop mutual goals.
  • Evaluate progression of care using evidence-based tools and escalate issues through the established chain of command.
  • Make referrals to third party payer and disease and case management programs as needed.
  • Facilitate patient throughput with a focus on effective care transitions.
  • Document care coordination interventions and case management activities.
  • Act as a liaison between physicians, patients, family, hospital staff, and outside agencies.
  • Seek ways to control costs without compromising safety and quality of care.
  • Direct activities for under-resourced patient populations, including education and community-based resources.
  • Participate in performance improvement activities and adhere to established policies and procedures.
  • Demonstrate knowledge of regulatory requirements and quality initiatives.
  • Advocate for patient's rights and ensure alignment with their values and preferences.

Experience: 2+ years in case management OR 3+ years in clinical nursing

Education: Associate Degree in Nursing or Nursing Diploma Required; Bachelor’s Degree in Nursing Preferred

License & Certification:

  • Registered Nurse
