Pay: $104,500 to $156,000 annually
Shift: Monday – Friday, 8 AM - 5 PM

Job Responsibilities
- Complete senior wellness visits at company-owned outpatient facilities and independently contracted physician offices.
- Conduct some home/custodial visits.
- Manage an average daily patient census of up to 16 patients, with 10-12 patients per day on average.
Required Skills
Core Skills
- Master’s degree from a four-year college and/or a professional certification beyond a four-year college.
- Certificate of completion from an accredited NP or PA Program.
- Unrestricted California State Medical License.
- Current California DEA certificate.
- EMR Proficient.
Preferred Skills
- Fluent or Proficient in Vietnamese or Spanish.
- 1+ year experience working in Primary Care.
- Previous HCC experience.
Requirements
Experience: 1+ year experience working in Primary Care.
Education: Master’s degree from a four-year college and/or a professional certification beyond a four-year college.
License & Certification:
- Unrestricted California State Medical License
- Current California DEA certificate
- Certificate of completion from an accredited NP or PA Program
