Job Responsibilities

Serve as a patient navigator throughout the continuum of care.
Function as a liaison for management staff, administration, physicians, managed care companies, community organizations, and other customers.
Coordinate efforts for treatment and promote quality improvement and educational efforts.
Compile data, track outcomes, and make recommendations for process improvement.
Provide referrals/support for follow-up care for patients who are uninsured or underinsured.

Required Skills

Core Skills

Excellent communication skills
Ability to work autonomously/independently

Preferred Skills

3-5 years of Oncology Nursing experience preferred
OCN/CBCN certified or willingness to obtain certification within 2 years of hire

Requirements

Experience: 3-5 years of Oncology Nursing experience preferred

Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.

License & Certification:

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required
Registered Nurse obtained prior to hire date or job transfer date required
