Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial, re-assessment, and focused assessment based on patient condition, setting, and population.
Identifies problems based on clinical judgment about the patient’s response to health conditions or needs.
Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Implements nursing care according to the care plan to ensure continuity of care.
Documents care in the patient’s record.
Ensures patient safety during medication administration and provision of treatments.
Educates patients and families regarding care.
Continuously evaluates the patient’s status and effectiveness of care, modifying the care plan as needed.
Coordinates care by managing and correcting the sequence, timing, and effectiveness of patient care from admission to discharge.
Delegates and supervises tasks as required.
Prioritizes nursing activities.
