Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial assessment.
Conducts re-assessment.
Conducts focused assessment based on patient condition, setting, and population.
Identifies problems based on clinical judgment regarding patient’s response to health conditions or needs.
Sets measurable and achievable short- and long-range goals in collaboration with patient and family.
Establishes daily goals with the patient and family.
Formulates a plan of care.
Implements nursing care according to the care plan.
Documents care provided in the patient’s record.
Ensures patient safety.
Administers medications and provides ordered treatments.
Educates patient and family about care.
Continuously evaluates patient’s status and effectiveness of nursing care.
Manages and corrects the sequence and timing of patient care from admission to discharge.
Delegates tasks as appropriate.
Supervises nursing staff.
Prioritizes tasks effectively.
