Job Responsibilities
- Perform and record patient history and physical examinations in accordance with hospital policy and standards.
- Establish differential diagnosis and communicate findings with the physician.
- Order appropriate lab and diagnostic procedures.
- Synthesize data to determine primary diagnosis and therapeutic plan utilizing established medical principles.
- Recognize and critically assess changes in patient’s condition, reporting to the hospitalist physician.
- Facilitate coordination and development of a comprehensive individual care plan in collaboration with patient, family, and multidisciplinary team.
- Facilitate one-on-one communication with primary care physician regarding current and ongoing patient care needs.
- Act as a hospitalist service liaison with patient care staff.
- Serve as a liaison between hospitalist, staff members, case managers, and attending physician regarding patients.

Requirements
Experience: Three to five years experience in acute care hospital medicine preferred.
Education: Completion of formal education program for Nurse Practitioner.
License & Certification:
- Current South Carolina Nurse Practitioner’s license with prescriptive authority
- ACLS Certification
