Job Responsibilities
- Act as the Skilled Nursing Facility’s leading provider of internal medicine services.
- Collaborate with the facility’s interdisciplinary team and physicians.
- Serve as a day-to-day clinical leader while helping oversee the care of all residents.
- Work with the facility’s entire patient population - short and long-term residents.
- Oversee day-to-day care with the assistance of nursing staff and guidance from medical directors.
- Evaluate new-admission visits.
- Coordinate discharge visits.
- Conduct monthly stability visits for long-term residents.
- Evaluate patients exhibiting changes in condition.
- Provide advanced care planning.
- Perform comprehensive health assessments and differential diagnosis.
- Prescribe treatments under protocol for acute and chronic illnesses.
- Prescribe necessary medications and treatments.
- Perform comprehensive physical examinations of patients.
- Develop and implement patient management policies and procedures.
- Facilitate referrals to other healthcare professionals and medical facilities.
- Communicate with attending physicians and/or specialists regarding patient care.
Required Skills
Core Skills
- Advanced Practice Licensure (APN) with a Florida license
- Active license and DEA number
- Experience in nursing or other medical fields
- Ability to build rapport with patients
- Thriving in a fast-paced environment
- Excellent written and verbal communication skills
- Strong leadership experience with teamwork emphasis
Preferred Skills
- Master’s degree
