Job Responsibilities
- Provide medication management and monitor patient responses to treatment.
- Collaborate with other healthcare professionals to coordinate care for patients with complex needs.
- Chart/schedule review.
- Determine appropriateness of patient and medication administration that day.
- Reach out to referring providers as necessary for questions/concerns related to patient condition and medication administration.
- Maintain a safe environment for patients during observation periods and monitor clinically per Sage Elevate protocols.
- Participate in specialized one-on-one training of other clinical staff as needed.
- Perform other duties as assigned.
