Job Title: Home Care Licensed Practical Nurse

Position: Performing nursing care in home settings

Shift: Tuesday 3pm-11pm, Wednesday 3pm-11pm, Thursday 6:30pm-11pm, Friday 6:30pm-11pm, Saturday 7am-9pm, Sunday 7am-9pm

Perform selected nursing procedures within the scope of LPN/LVN practice, including tracheostomy care, colostomy care, enemas, douches, catheterizations, dressing changes, and phlebotomies.
Provide patient and family teaching according to the plan outlined by the RN.
Monitor patient's vital signs and make specific measurements and observations.
Monitor and record intake and output; report significant changes immediately.
Observe, assess, and record the patient's physical and mental condition and report any changes.
Administer medications as instructed by the physician and RN; document the patient's file accordingly.
Work effectively with patients, families, and health care team members to coordinate patient care.
Maintain a high degree of confidentiality due to access to sensitive information.
Maintain regular, predictable, consistent attendance and be flexible to meet the needs of the agency.
Follow all Medicare, Medicaid, and HIPAA regulations and requirements.
Abide by all regulations, policies, procedures, and standards.
Perform other duties as assigned.

Ability to complete necessary documentation appropriately, legibly, and timely

Reliable transportation, current driver's license, and appropriate automobile insurance; or ability to take public transportation

Experience: At least two (2) years of clinical practical nursing experience

Education: Graduate from an accredited school of practical nursing
