Job Responsibilities
- Acts as a first-line manager for designated staff and assures coordination of new referrals and patient care.
- Oversees the development, implementation, and updates of the individualized plan of care.
- Manages day-to-day provider issues and care delivery.
- Contributes to improving quality and consistency of patient care and experience.
- Evaluates quality of care, patient satisfaction, and compliance, and oversees patient and personnel assignments.
- Reviews, implements, and interprets clinical policies and procedures as directed by the Director of Clinical Services (DOCS).
- Serves as a clinical lead at relevant clinical and departmental meetings.
- Enforces and demonstrates compliance with all State and Federal regulations.
- Supports and executes the mission, ethics, and goals of the company effectively.
- Represents themselves in a positive and professional manner in the company and community.
- Adheres to dress code with a clean and neat professional appearance.
- Reports as scheduled to complete work within the designated time.
- Adheres to all company policies and procedures outlined in the Employee Handbook.
- Aids in terminations, coaching, education, and counseling of field staff.
- Ensures supervision of all home care personnel, as assigned.
- Ensures effective communication to coordinate the implementation of each patient’s Plan of Care.
- Instructs, trains, and re-educates on clinical skills/procedures when necessary.
- Provides support, guidance, and education to patients and families to maintain optimal well-being.
- Communicates all occurrences and relevant information to the Clinical Supervisor/DOCS.
- Ensures any observed or suspected child/adult abuse, neglect, or exploitation is reported to proper authorities.
- Collaborates with Clinical Supervisor and DOCS to resolve client/clinical concerns timely.
- Participates in agency-sponsored continuing education and training to maintain clinical competency.
- Develops and implements treatment plans under the direction of physicians.
- Completes all clinical documentation timely, ensuring it meets agency policies and State regulations.
- Reviews clinical documentation for completeness and to meet payer source requirements.
- Interviews and evaluates potential clinical field staff for competency.
- Complies with and participates in continuing quality improvement initiatives.
- Runs clinical meet and greets for potential Spanish-speaking PDN and HHA families.
