Job Responsibilities

  • Acts as a first-line manager for designated staff and assures coordination of new referrals and patient care.
  • Oversees the development, implementation, and updates of the individualized plan of care.
  • Manages day-to-day provider issues and care delivery.
  • Contributes to improving quality and consistency of patient care and experience.
  • Evaluates quality of care, patient satisfaction, and compliance, and oversees patient and personnel assignments.
  • Reviews, implements, and interprets clinical policies and procedures as directed by the Director of Clinical Services (DOCS).
  • Serves as a clinical lead at relevant clinical and departmental meetings.
  • Enforces and demonstrates compliance with all State and Federal regulations.
  • Supports and executes the mission, ethics, and goals of the company effectively.
  • Represents themselves in a positive and professional manner in the company and community.
  • Adheres to dress code with a clean and neat professional appearance.
  • Reports as scheduled to complete work within the designated time.
  • Adheres to all company policies and procedures outlined in the Employee Handbook.
  • Aids in terminations, coaching, education, and counseling of field staff.
  • Ensures supervision of all home care personnel, as assigned.
  • Ensures effective communication to coordinate the implementation of each patient’s Plan of Care.
  • Instructs, trains, and re-educates on clinical skills/procedures when necessary.
  • Provides support, guidance, and education to patients and families to maintain optimal well-being.
  • Communicates all occurrences and relevant information to the Clinical Supervisor/DOCS.
  • Ensures any observed or suspected child/adult abuse, neglect, or exploitation is reported to proper authorities.
  • Collaborates with Clinical Supervisor and DOCS to resolve client/clinical concerns timely.
  • Participates in agency-sponsored continuing education and training to maintain clinical competency.
  • Develops and implements treatment plans under the direction of physicians.
  • Completes all clinical documentation timely, ensuring it meets agency policies and State regulations.
  • Reviews clinical documentation for completeness and to meet payer source requirements.
  • Interviews and evaluates potential clinical field staff for competency.
  • Complies with and participates in continuing quality improvement initiatives.
  • Runs clinical meet and greets for potential Spanish-speaking PDN and HHA families.
