Job Responsibilities

Acts as a first-line manager for designated staff and assures coordination of new referrals and patient care.
Oversees the development, implementation, and updates of the individualized plan of care.
Manages day-to-day provider issues and care delivery.
Contributes to improving quality and consistency of patient care and experience.
Evaluates quality of care, patient satisfaction, and compliance, and oversees patient and personnel assignments.
Reviews, implements, and interprets clinical policies and procedures as directed by the Director of Clinical Services (DOCS).
Serves as a clinical lead at relevant clinical and departmental meetings.
Enforces and demonstrates compliance with all State and Federal regulations.
Supports and executes the mission, ethics, and goals of the company effectively.
Represents themselves in a positive and professional manner in the company and community.
Adheres to dress code with a clean and neat professional appearance.
Reports as scheduled to complete work within the designated time.
Adheres to all company policies and procedures outlined in the Employee Handbook.
Aids in terminations, coaching, education, and counseling of field staff.
Ensures supervision of all home care personnel, as assigned.
Ensures effective communication to coordinate the implementation of each patient’s Plan of Care.
Instructs, trains, and re-educates on clinical skills/procedures when necessary.
Provides support, guidance, and education to patients and families to maintain optimal well-being.
Communicates all occurrences and relevant information to the Clinical Supervisor/DOCS.
Ensures any observed or suspected child/adult abuse, neglect, or exploitation is reported to proper authorities.
Collaborates with Clinical Supervisor and DOCS to resolve client/clinical concerns timely.
Participates in agency-sponsored continuing education and training to maintain clinical competency.
Develops and implements treatment plans under the direction of physicians.
Completes all clinical documentation timely, ensuring it meets agency policies and State regulations.
Reviews clinical documentation for completeness and to meet payer source requirements.
Interviews and evaluates potential clinical field staff for competency.
Complies with and participates in continuing quality improvement initiatives.
Runs clinical meet and greets for potential Spanish-speaking PDN and HHA families.
