Job Responsibilities
- Supervises daily operations of discharge planning and social services in the inpatient and emergency department.
- Direct supervision of Care Management staff.
- Acts as primary point of contact for inpatient unit Care Management related barriers/concerns.
- Manages own assignment and patient/department outcomes of those under direct supervision.
- Assists Care Management Manager/Facility Director in implementing Corporate Care Management strategies.
- Ensures active staff participation in daily Multidisciplinary Rounds.
- Serves as a point of contact for all identified stakeholders.
- Acts as a content resource regarding medical necessity criteria, patient status, and discharge criteria.
- Assures staff documentation is completed per guidelines and coaches staff as needed.
- Facilitates care conferences for complex transitions and/or placements.
- Participates in hospital/medical staff meetings to review patients meeting criteria related to high dollar/extended length of stay and/or difficult placement.
- Reports pertinent quality/risk issues to appropriate individuals, departments, and/or committees in a timely manner.
- Discusses patient, staff, physician, and other department complaints with Care Management Manager/Facility Director and follows up within 24 hours.
- Demonstrates a positive problem-solving attitude.
- Serves as a supportive change agent for Care Management model processes.
- Acts as a role model for Care Management department staff.
- Encourages team member engagement.
- Maintains open communication with staff.
- Assists with staff performance reviews/evaluations and provides support with corrective action plans.
- Balances schedules to meet department needs prior to posting.
- Communicates changes to expected schedule and minimizes unscheduled absences.
- Plans job responsibilities for maximum utilization of staff.
- Resolves daily staffing issues.

Preferred Skills
- CCM, ACM, or RN-BC certification in Care Management
- Advanced degrees (BSN or MSW)
Requirements
Experience: Two (2) years of experience in direct Care Management
Education: Associates Degree in Nursing or BSW
License & Certification:
- State specific RN license if a nurse
