Supervise, evaluate, and provide nursing services to clients in various senior living communities.
Provide 1:1 patient care, following each patient from start to discharge.
Collaborate with facility wellness staff to ensure strong lines of communication and continuity of care.
Consult with physicians and the office regarding patient care, providing necessary details.
Act as the point of contact for all disciplines involved with providing care to patients.
Perform accurate OASIS collection.
Ensure the medication profile remains current.
Ensure lab values are reported to physicians in a timely manner.
Attend weekly case conferences and monthly case manager meetings.

Experience: Minimum of one year of clinical experience preferred
