Job Responsibilities
- Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
- Partner closely with the Clinical Director to assess the eligibility and suitability of clients for hospice services.
- Coordinate care and provide comfort and support for patients to ensure their quality of life.
- Deliver care rooted in proven nursing principles and techniques to help ease pain and bring solace.
- Educate and empower patients and families to encourage their active participation in personalized care plans.
- Share the responsibility of call duties with the team.
Required Skills
Core Skills
- Current unencumbered license as a Registered Nurse
- Minimum one year experience as an RN in a medical surgical/acute care setting
- Computer proficiency including EMR
- Current CPR certification if required by state
Preferred Skills
- Experience in hospice care
Requirements
Experience: Minimum one year experience as an RN in a medical surgical/acute care setting; hospice experience is a plus.
