Job Responsibilities
- Use the nursing process to address the physical, psychological, emotional, and social needs of patients.
- Collaborate with providers and support staff to provide clinical nursing interventions.
- Communicate effectively and respectfully with patients to address their individualized needs and priorities.
- Integrate evidence-based practice and patients' cultural beliefs for optimal healthcare delivery.
- Collaborate and communicate with primary care providers regarding patient care and management.
- Support the implementation of clinical care pathways for common primary care conditions.
- Utilize evidence-based ambulatory care nursing interventions during clinical visits and communications.
- Provide education to patients and their families on preventive care and chronic disease management.
- Document all patient interactions in the electronic medical record (EMR).
- Perform care coordination for patients including abnormal results and management of vaccinations.
- Oversee clinical assessment, promotion, maintenance, and restoration of patients’ health.
- Minimize risk of harm by following established nursing practices and protocols.
- Use information technology to manage knowledge and mitigate errors.
- Monitor care outcome processes and support ethical decision-making.
- Function within inter-professional teams to foster communication and mutual respect.
- Create an environment of belonging and inclusion.
- Demonstrate commitment to learning and seek opportunities for professional growth.
- Participate in team huddles to provide updates on clinical operations.
- Demonstrate a willingness to give and receive feedback for practice integration.
Required Skills

- RN licensure in state of practice

Requirements
Experience: 1+ years’ experience as a Nurse in an ambulatory setting preferred
Education: Graduate of an accredited school of nursing required; BSN or completion of BSN strongly preferred
License & Certification:
- Current RN licensure
- BLS certification
