Job Responsibilities

Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts initial assessment, re-assessment, and focused assessments based on patient condition, setting, and population.
Identifies problems related to patient’s response to actual or potential health conditions or needs.
Plans and sets measurable and achievable goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Formulates a plan of care and transition or discharge planning.
Implements nursing care according to the care plan to ensure continuity of care.
Documents patient care in healthcare record.
Ensures patient safety.
Administers medication and provides ordered treatments.
Educates the patient and family about care.
Evaluates patient status and effectiveness of nursing care continuously.
Modifies care plan as needed.
Coordinates care with patients, families, and/or healthcare team members from admission to discharge.
Delegates, supervises, and prioritizes care as necessary.

Valid current Arizona Nursing License or compact state license
Basic Life Support (BLS) Certification
Nursing care planning and assessment skills

Preferred Skills

Advanced Cardiac Life Support (ACLS) Certification
Pediatric Advanced Life Support (PALS)
Neonatal Resuscitation Program (NRP)

Requirements

Experience: None required

Education: Graduation from an accredited School of Nursing

License & Certification:

Valid current Arizona Nursing License or compact state license
Basic Life Support (BLS) Certification
Advanced Cardiac Life Support (ACLS) Certification preferred
