Job Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Conducts systematic assessments, including initial, re-assessment, and focused assessment.
- Identifies problems based on clinical judgment regarding patient’s health conditions.
- Plans measurable and achievable short- and long-range goals in collaboration with the patient and family.
- Establishes daily goals with the patient and family.
- Formulates a plan of care and ensures continuity of patient care.
- Documents patient care and evaluations in healthcare records.
- Ensures patient safety through medication administration and provision of ordered treatments.
- Educates patients and families on care processes.
- Coordinates care actions with patients, families, and healthcare team members.

Requirements
Experience: None required
Education: Graduation from an accredited School of Nursing
License & Certification:
- Valid current Arizona Nursing License or compact state license
- Basic Life Support (BLS) - American Heart Association
