Job Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Uses a systematic approach to collect and analyze data about a patient.
- Conducts initial, re-assessment, and focused assessments based on patient condition.
- Identifies problems and sets measurable and achievable goals in collaboration with the patient and family.
- Establishes daily goals with the patient and family.
- Formulates a plan of care.
- Implements nursing care according to the care plan to ensure continuity of care.
- Documents patient care in the healthcare record.
- Continuously evaluates patients’ status and the effectiveness of nursing care.
- Implements care coordination actions to manage and correct the sequence, timing, and effectiveness of patient care.
- Delegates and supervises nursing staff as necessary.
- Prioritizes patient care tasks.

Requirements
Experience: None required
Education: Graduation from an accredited School of Nursing
License & Certification:
- Valid current Arizona Nursing License
- Basic Life Support (BLS) - American Heart Association
