Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Conducts systematic and dynamic assessments, including physiological, psychological, sociocultural, spiritual, economic, and lifestyle factors.
Sets measurable and achievable short- and long-range goals in collaboration with the patient and family based on assessment and problem identification.
Documents care in the patient’s record to ensure continuity of care during hospitalization and discharge preparation.
Ensures patient safety through effective medication administration and provision of ordered treatments.
Provides patient/family education and nursing care.
Continuously evaluates the patient’s status and the effectiveness of nursing care, modifying the care plan as needed.
Manages and corrects the sequence, timing, and effectiveness of patient care from admission to discharge.
