Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Collects and analyzes data about a patient using a systematic approach.
Conducts initial assessment, re-assessment, and focused assessment based on patient condition.
Sets measurable and achievable goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Formulates a plan of care and ensures continuity of care during hospitalization and discharge preparation.
Documents care provided in the healthcare record.
Ensures patient safety and medication administration.
Provides patient/family education and nursing care.
Continuously evaluates patient status and the effectiveness of nursing care.
Manages care coordination with patients, families, and healthcare team members.
Delegates and supervises nursing tasks as necessary.
Prioritizes tasks to ensure effective patient care.
