Job Responsibilities
- Assist with all clinical needs for Beaufort patients.
- Ensure seamless coordination of patient care.
- Prepare charts proficiently.
- Communicate effectively with patients and providers.
- Schedule patient visits.
- Utilize and log chronic care management activities appropriately.
- Provide support to providers and patients.
- Assess vital signs and perform hands-on tasks with patients.
