Job Responsibilities
- Assess transitional care needs and coordinate care across the continuum.
- Engage with patient and family to assure care needs are met.
- Manage length of stay and promote efficient utilization of resources.
- Perform psychosocial and functional status assessments.
- Plan transitional care and clinical care progression.
- Facilitate patient/family care conferences.
- Participate in interdisciplinary rounds.
- Educate patients and families on care plans.
- Collaborate with utilization review nurse, patient's physicians, and interdisciplinary care team to assess medical and psychosocial needs.
- Ensure patient progresses toward discharge goals and identify barriers.
- Consult with appropriate disciplines to resolve care delays.
- Manage all aspects of discharge planning for assigned patients.
- Develop individualized discharge plans with patients and families.
- Identify and document barriers for timely patient disposition.
- Participate in Interdisciplinary Rounds to confirm discharge estimates and transitional care recommendations.
- Document assessments and care plans in medical records.
- Complete all required professional competency assessments and education.

Requirements
Experience: Minimum 1 year nursing experience in the acute care setting.
Education: Associates in Nursing
License & Certification:
- Registered Nurse (Single State) or RN - Multi-state Compact
- Basic Life Support (BLS) - Instructor
