Job Responsibilities

Coordinate and oversee the interdisciplinary assessment and care planning process in the center
Conduct comprehensive resident assessments and care plans
Maintain regular communication among care team members, the resident, and family
Collect and transmit data for quality improvement purposes
Adhere to the Minimum Data Set (MDS) and Resident Assessment Instrument (RAI) requirements
Float to the floor in times of need at the center

MDS experience
Nursing qualifications (RN or LPN)

Preferred Skills

Current MDS 3.0 certification

Requirements

Experience: Minimum of 1+ years of MDS experience in a Skilled Nursing Facility

License & Certification:

Registered Nurse (RN) license in Illinois
Licensed Practical Nurse (LPN) license in Illinois
