Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Collects and analyzes data about a patient during the assessment phase.
Identifies problems related to the patient's response to health conditions or needs.
Develops measurable and achievable goals in collaboration with the patient and family.
Establishes daily goals with the patient and family.
Implements nursing care according to the care plan and documents care in the patient's record.
Ensures patient safety and administers medications.
Provides treatments as ordered and educates patients/families.
Continuously evaluates the patient’s status and the effectiveness of nursing care.
Manages care coordination from admission to discharge.
