Implements and monitors patient care plans.
Monitors, records, and communicates patient condition as appropriate.
Serves as a primary coordinator of all disciplines for well-coordinated patient care.
Notes and carries out physician and nursing orders.
Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
Collects and analyzes data about a patient, including physiological, psychological, sociocultural, spiritual, economic, and lifestyle factors.
Establishes measurable and achievable short- and long-range goals in collaboration with the patient and family.
Documents care in the patient's record.
Ensures patient safety through medication administration and provision of ordered treatments.
Educates patients and families.
Continuously evaluates the patient's status and effectiveness of nursing care.
