Job Responsibilities
- Collect pertinent data and information relevant to the healthcare consumer's health or situation.
- Analyze assessment data to determine actual and potential diagnoses or issues.
- Identify expected outcomes for a plan individualized to the healthcare consumer or the situation.
- Develop and modify a plan of care that prescribes strategies and alternatives to attain expected outcomes.
- Implement the identified plan.
- Coordinate care delivery and transitions in care.

Requirements
Experience: Registered Nurse is the foundational job description for all RNs; all RNs are encouraged to advance through the SPIRE clinical advancement program.
Education: Graduation from an accredited school of nursing; BSN preferred.
License & Certification:
- Licensed and a currently registered nurse in the State of Connecticut
- BCLS certification within one month of hire
