Job Responsibilities

Provide therapeutic patient and family centered care.
Collect and assess data related to patient health status.
Develop, execute, and evaluate the plan of care.
Utilize evidence to drive clinical decisions and practice.
Establish goals and strategies for care coordination.
Consider safety, effectiveness, and fiscal accountability in care delivery.
Delegate and supervise care according to the WI Nurse Practice.
Participate in continuous quality improvement initiatives.
Assume responsibility for professional development.
Coordinate patient care and provide direct patient care.
Educate patients and conduct telephone triage/eVisits.
Perform and assist with in-clinic minor procedures.
Float to other areas as needed for operational needs.
Complete other duties as assigned.

Required Skills

Core Skills

Clinical experience in patient care management
Nursing assessment and data collection skills
Patient and family education

Preferred Skills

At least 1 year of relevant nursing experience
Bachelor's Degree in Nursing

Requirements

Experience: At least 1 year of relevant nursing experience, preferred.

Education: Bachelor's Degree in Nursing, preferred.

License & Certification:

Current state of Wisconsin Registered Nurse License
Multi-state Nursing License from a participating state in the NLC
American Heart Association (AHA) Basic Life Support for Healthcare Provider
