Job Responsibilities
- Perform client visits and coordination of client care.
- Act as the point of contact for all disciplines involved with patient care.
- Oversee the frequency of visits for the episode.
- Consult with the physician and office regarding patient care details.
- Perform accurate OASIS collection.
- Ensure the medication profile remains current.
- Ensure lab values are reported to the physician in a timely manner.
- Attend weekly case conferences and monthly case manager meetings.
Required Skills
Core Skills
- Registered Nurse (RN) licensed in the state of employment
- Clinical experience in nursing
- Knowledge and skill in current nursing practice
- Valid state driver's license
- Automobile liability insurance
Preferred Skills
- One year of clinical experience preferred
