Assist other health care members in providing patient specific detailed education regarding adequacy measures.
Welcome assigned patients and inquire as to their well-being since their last treatment.
Evaluate vascular access for patency, perform vascular access cannulation, and administer heparin as allowed by state law.
Obtain pre and post treatment vital signs and weight, and perform vascular access evaluation pre-treatment.
Initiate dialysis treatment according to prescribed orders including blood flow and dialysate flow.
Monitor patients’ responses to dialysis therapy and document all relevant functions for treatment discontinuation.
Evaluate the patient prior to discharge and obtain hemostasis with appropriate dressings.
Ensure appropriate safety alarms are enabled on dialysis machines.
Document treatment data into clinical applications accurately and timely.
Assist with clinical orientation and ongoing training of staff.
Participate in the recruitment process for new clinical staff.
Conduct regular Continuous Quality Improvement activities.
Assist with infection control audits and reinforce adherence to infection control practices.
Follow all newly admitted patients through their first 4 weeks of dialysis and coordinate transitional dialysis care.

Experience: At least five years patient care experience in a hospital setting or a related facility preferred.

Education: High School diploma or G.E.D. required; Bachelor’s degree preferred.
