Job Responsibilities
- Implements and monitors patient care plans.
- Monitors, records, and communicates patient condition as appropriate.
- Serves as a primary coordinator of all disciplines for well-coordinated patient care.
- Notes and carries out physician and nursing orders.
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
- Conducts initial assessments, re-assessments, and focused assessments based on patient condition.
- Identifies problems related to patient’s response to health conditions or needs.
- Sets measurable and achievable short- and long-range goals in collaboration with the patient and family.
- Establishes daily goals with the patient and family.
- Formulates a plan of care and coordinates transition or discharge planning.
- Implements nursing care according to the care plan to ensure continuity of care.
- Ensures patient safety and medication administration.
- Provides education to patients and families.
- Documents care in the healthcare record.
- Continuously evaluates the patient’s status and effectiveness of nursing care.
- Manages and corrects the sequence, timing, and effectiveness of patient care from admission to discharge.
- Delegates and supervises care appropriately.
- Prioritizes patient care needs.

Requirements
Experience: None
Education: Graduation from an accredited School of Nursing
License & Certification:
- Valid current Arizona Nursing License or compact state license
- Basic Life Support (BLS) - American Heart Association
