Job Responsibilities
- Provision of exceptional primary care.
- Conduct office visits for routine and acute issues.
- Administer Annual wellness visits and health risk assessments, focusing on thoughtful, accurate documentation.
- Care coordination with other providers, specialists, testing facilities, and agencies.
- Population health leadership, ensuring eligible females receive evidence-based breast cancer screenings every 2 years.
- Assist the care team with phone triage and outreach.
- Educate patients on health conditions, care plans, and treatments.
- Participate in promotional activities for Oak Street Health.
- Conduct home visits as needed.
- Other duties as assigned.
Required Skills
Core Skills
- Master of Science in Nursing or Doctor of Nursing Practice
- National certification as a Nurse Practitioner
- Active Nurse Practitioner license
- Active DEA license
Preferred Skills
- Experience in primary care: internal medicine, geriatrics, or family medicine
- Understanding of HCC documentation, ICD-10 Coding, and Health Risk Assessments
- Bilingual proficiency
- Passion for teamwork and collaboration
Requirements
Experience:
Education: Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
License & Certification:
- National certification as a Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
- Active DEA license
