Job Responsibilities

Provide patient centered clinical case management to support health and health-related outcomes for individuals with complex health and social needs.
Conduct community-based case management and field-based visits in various settings, including private apartments, motels, correctional facilities, and shelters.
Work closely with a diverse group of partners and providers in the health and social services arena, including state partners.
Utilize a team-based care approach, patient engagement tools, shared care plans, and motivational interviewing skills.
Foster effective communication and working relationships with individuals, family members, health care disciplines, and service providers.

Requirements

Experience: Three (3) years or more of professional nursing experience in an acute hospital setting, long term care, health insurance carrier, or within a community health/public health setting.

Education: Eligible for or possession of licensure as a Registered Nurse (RN) in Vermont.

License & Certification:

Registered Nurse (RN) in Vermont or eligible via a multi-state license

Schedule:

Flexible to cover other counties if requested
Certain functions may require duty outside of normal work hours
